This is to inform all newly admitted College of Medical Sciences students that they are to follow the below Procedures for their registration;
1. Check for your name on the Admission list available at the following sites:
a) Notice board b) Web site:www.unimaid.edu.ng
2.Collect your Admission letter at the College of Medical Sciences from Wednesday, 28th July, 2021. following documents for this:
You will need the a) JAMB Result/Original JAMB Slip
b) Original Statement of Result.
c) Payment of College Prospectus Fee.
3. Venue for the Registration: Lecture Theatre and ETF Classes in the College of Medical Sciences
a) Write your name on your Departmental List
4. Report to the Board Room at the College of Medical Sciences for Screening. You are to proceed to the Head of your Department and the Chairman Screening Committee with the following Documents:
a) Original Admission Letter
b) Two (2) duly completed Screening forms with Two (2) recent passport size photographs.
c) 2 duly completed clearance form
d) JAMB online Result.
e) University online Post UTME Screening Form.
f) Original copy of your credentials (NECO, GCE and SSCE) must not be more than Two (2) sittings
g) For any “O” level statement of Results from 2018 and below must be accompanied with a Certificate. While for those obtained from 2019 to date should be a statement of results accompanied by an online copy of the results and scratch cards for verification of the Results
5. Proceed to the Registration Officer for the following
a) ID Number.
b) Schedule of fees.
c) Submit Photocopies of your Admission Letter, JAMB Result, Screening, and Clearance Forms to the Registration Officer.
d) Submit Photocopies of your Admission Letter and JAMB Result to the Internet Cafe for Upload of your Registration.
e) Go to the designated Bank for the College and pay the Full Fees and Charges
The Screening and Registration commence on Monday 2nd August, 2021 at 9:00am in the College Board Room, prior to this students would be arranged on first come first serve basis in the Lecture Theatre and ETF Classes. The cooperation of all would be required for this purpose, please.
